Graduate Student Forms

All forms are fillable and electronic signatures are accepted.

Please obtain all required signatures before submitting to the Graduate School. Completed forms may be submitted in-person or by email.

General Request Forms

Degree Audit Change Form - Used to request a change to an existing degree audit for a graduate student.
Request for Dual Degree - Used to request enrollment in a dual degree program.
Request for Change of Department - Used to formally request a transfer to a different academic department.
Request for Change of Degree Program - Used to request a switch to a different degree program.
Request for Course Pass/Fail - Submitted to request that a course be graded on a pass/fail basis instead of a letter grade
Request to Change Defense Date - Used to request a change to a previously scheduled thesis or dissertation defense date.
Request for Degree Candidate Deletion - Used to remove a student from the degree candidacy list for a given term.
Louisiana Residency Appeal Form - Used to appeal a student's Louisiana residency classification for tuition purposes.

Certificate Program Student Forms

Application for Degree: Graduate Certificate - Submitted by students to formally apply for conferral of a graduate certificate.
Application for Degree: Certificate of Education Specialist - Submitted by students to formally apply for conferral of a Certificate of Education Specialist.

Degree Audit Forms

Graduate Certificate Audit - Used to verify that all coursework requirements for a graduate certificate have been met.
Certificate of Education Specialist Degree Audit - Used to verify that all coursework requirements for the Education Specialist certificate have been met.

Master's Degree Student Forms

Application for Degree: Master's Degree - Submitted by students to formally apply for conferral of a master's degree.
Course Revalidation - Used to request revalidation of coursework that has exceeded the time limit for degree credit.
Request for the Accelerated Master's Program - Used to apply for admission into an accelerated (combined bachelor's/master's) program.
Request for Graduate Credit for LSU Seniors - Allows LSU seniors to request that certain courses count toward both undergraduate and graduate credit.
Request for Transfer Credit - Used to request that credit from another institution be applied toward a graduate degree.
Verification of Graduate Concentration - Used to confirm completion of requirements for a declared graduate concentration.

Degree Audit Forms

Master's Degree Audit* - Verifies completion of degree requirements for select master's programs.
*NOTE: This form is ONLY required for applicants in the following programs: Accounting, Business Administration, Finance, Human Resource and Leadership Development, Landscape Architecture, School of Library and Information Studies, and Social Work (Non-Thesis).
Request for Master’s Defense and Degree Audit - Used to schedule a master's thesis defense and initiate the final degree audit.

Doctoral Degree Student Forms

Application for Degree: Doctoral Degree - Submitted by students to formally apply for conferral of a doctoral degree.
Request for Final Defense - Used to schedule and request approval for a doctoral candidate's final dissertation defense.
Survey of Earned Doctorates - A federally required survey completed by doctoral candidates upon completion of their degree.
Declaration of Co-Authorship - Used to disclose and document co-authorship of material included in a dissertation.
Verification of Graduate Concentration - Used to confirm completion of requirements for a declared doctoral concentration.

Request for Doctoral General Defense and Degree Audit - Used to schedule a doctoral general exam/defense and initiate the degree audit.

Online Student Forms

Application for Degree: Online Graduate Certificate - Submitted by online students to formally apply for conferral of a graduate certificate.
Application for Degree: Online Masters Program - Submitted by online students to formally apply for conferral of a master's degree.

Degree Audit Forms

Online Master's Program Degree Audit** - Verifies completion of degree requirements for select online master's programs
**NOTE: This form is ONLY required for applicants in the following programs: Analytics, Civil Engineering, Computer Science, Construction Management, Leadership and Human Resource Development, Liberal Arts, Library & Information Science (MLIS), MBA, Social Work (Non-Thesis). If you are majoring in any other field, please disregard this form, including the signatures listed.
Online Graduate Certificate Degree Audit - Used to verify that all coursework requirements for an online graduate certificate have been met.
Request for Master's Defense and Degree Audit (Online) - Used by online master's students to schedule a defense and initiate the final degree audit.
